If it is on only one side, then x rays are in order. When the big toe is longer, the first metatarsal is longer, and he should probably have inserts to make sure the weight distribution is even in this foot. A long first metatarsal typically means more pressure on the ball of the foot. See a podiatrist for a discussion. Hope this helps some.
